Foundation Health

Foundation Health is a membership-based direct primary care practice focused on improving access to its care team. The practice includes an initial visit based on a prior health assessment, blood draws for initial health testing, and follow-up health analysis to review results and develop a personalized wellness plan. Foundation Health also works with wellness partners, including acupuncturists and massage therapists, to help patients access additional services.
